Another extensive census-based study of Muslim populations in Uganda (n = 80,000) and Iraq (n = 250,000) demonstrated that exposure to Ramadan fasting during the first month of pregnancy increased the risk of mental or learning disability in offspring by 50% and psychological disability by 63%! Additionally, a large study on Indonesian women indicated that exposure to Ramadan during pregnancy resulted in smaller statures and thinness in adulthood. Lower birth weight is directly linked to impaired cognitive function. For example, research has shown that Muslim children exposed to Ramadan fasting in utero during the first trimester had significantly lower scores in maths, reading, and writing tests (taken in primary schools at the age of 7 years) than Muslim children who were not exposed to Ramadan fasting in utero.

General Procedure
Length
Intermittent fasting
- Aka time restricted fasting/ feeding.
- Eat only within an 8hr window. Fast the remaining 16 hours.
- Minimize muscle loss
- Excercise (esp strength training) shortly before breaking fast.
Periodic fasting
- Fasting 2 to 5 days straight. Mark Baker recommends 2+ weeks inbetween.
Activity

Sustenance
- In case of tiredness, just lie down for a few minutes, or sit upright.

During long fasts
- Vijay uses rasam to keep up electrolytes; buttery coffee to avoid gall stones.

Breaking fast

Diarrhea after breaking fast
- it’s common to experience diarrhea after breaking a fast because your bowel’s ability to function properly decreases when it’s not used. When you start eating again, the sudden influx of food can overstimulate the gastrocolic reflex, which can lead to diarrhea.
- This can be made worse by eating certain foods, such as those that are high in sugar (oft sucrose or frutctose) or fat. These foods can cause “dumping syndrome,” where food is pushed into the small intestine before it’s properly broken down, which can lead to diarrhea.
- Symptoms of dumping syndrome usually occur within minutes after eating.
Timing
- “Most people start the timing of their fast directly after their last meal, i.e. the post-prandial state, when the stomach and intestines are absorbing nutrients from food. However, it’s only after complete digestion and absorption of a meal (Post prandial state), 4-6hrs late that a fast actually begins.”

Contra-indications
- Post nasal drip into stomach stimulates acid secretion. Plus good nutrition is required to fight viral infections. Hence, don’t fast in such situations.

Stages
- Post prandial state: After eating, when the stomach and intestines are absorbing nutrients from food. 4-6hrs.
- glycolysis, the body normally derives energy from glucose in the blood stream which comes from carbohydrate in food.
- Glucose which isn’t used straightaway is converted into glycogen and stored in the liver and muscles.
- post-absorptive state: body has to create energy from the breakdown of its own reserves.
- hepatic glycogen is converted back into glucose for energy.
- Once the glycogen stores are near depletion, 12-24hrs later depending on activity levels, the body creates glucose/energy from non-carbohydrate sources such as fat and amino acids (gluconeogenesis).
- After 2-3 days of fasting the body switches’ to the muscle sparing process of lipolysis/ketosis.
Muscle loss
- Fasting results in muscle loss. Even intermittent fasting - especially if exercising-breakfast gap is long. (PA2022)

- Obese individuals will retain muscle longer than lean people (due to higher non-muscle Pr stores)
- 5+ days on, you will start to lose some muscle

Adipose tissue loss
- white adipose tissue (body fat) makes and releases leptin. Leptin helps inhibit (prevent) hunger and regulate energy balance so that your body doesn’t trigger a hunger response when it doesn’t need energy (calories). Leptin doesn’t affect your hunger levels and food intake from meal to meal but rather acts to alter food intake and control energy expenditure over a longer period of time to help maintain your normal weight.
- As your body fat (adipose tissue) decreases, your leptin levels decrease, which signals your body to think that it’s starving. This stimulates intense hunger and appetite and can lead to increased food consumption.
Gall stones
While short fasting periods (around 10 hours) may be safe, longer fasting durations (up to 20 hours) might be beneficial in reducing gallstone risk after an initial increase of risk at 15 hours.
About 10g of fat a day is supposed to cause enough gallbladder contractions to empty the bile
Initiation
It is actually pretty easy to start off fasting habit early in life. For example, for over a year, 6yo son has been happily doing daily (4:30PM-8:30AM) intermittent fasting and ~24 hour fast on ekAdashi-s (albeit cheating with milk and fruits), while staying fully active (eg 1 hr swimming + 1 hr dance).
